Botox
Facial lines are caused by the repeated use of the facial muscles in a particular manner. Just as a freshly ironed shirt will become severely creased, for example at the elbow as a result of bending the arm, in a similar way our facial lines form whenever we frown, squint or smile. Some people have a distinctive facial habit — grimacing, raising eyebrows, using one side of the face more than the other — and in time they will see a set of facial lines appear which reflect this individual habit.

Lines can be softened or in some cases erased by means of ‘filler injections’ or surgical/laser peel treatment. These treatment processes vary as to how effective they are and how long they last, however, they all have one common characteristic: the effect is being treated rather than the cause. Unsightly facial lines have already been formed and these are continually reinforced every time we frown or squint.

The Medical Aesthetic Clinic’s ‘Hibernation’ Treatment is different in that it is a preventative approach. Injections are given, usually to the forehead frown area, which temporarily weakens the frown muscles and thus puts them to sleep for a period of 3 to 6 months. The weakening of the muscles is always temporary and can never be permanent which is why the treatment is called ‘The Hibernation Treatment’ since the muscle temporarily ‘hibernates’.

The basic aim is to break the habit of frowning and educate the patient in their facial movements so that in effect the patient forgets how to frown. Usually a patient will have several of these injections over a number of years until the frowning habit is broken.

It is generally agreed that frowning is an unpleasant habit, which tends to spoil the serenity and beauty of the face. Facial lines can sometimes be markedly lopsided due to overactive muscles on one side of the face. This treatment can be used to selectively weaken such muscles thereby balancing the resting tone for both sides of the face.
Detailed Information

The injection procedure takes about 5 to 10 minutes. The practitioner will ask you to frown hard, so that the exact pattern of forehead and frown movement is clearly seen for each individual patient. The injections are slightly uncomfortable but quite brief. For example, the Hibernation Treatment is less sore than ‘Filler’ injections.
​
Afterwards, there will be some tiny red marks for approximately one hour where the micro-bore needle was inserted. The injection starts to take effect after 2 to 4 days and you will feel that it is becoming more difficult to frown. After about two weeks the maximum effect will be apparent.
​
Very occasionally an individual patient may require more than the usual injection amount and a ‘top-up’ injection may be required. Some patients who have well exercised and developed frown muscles (more common in men) may need a second injection. It is good clinical practise to inject a conservative amount of the treatment and then add more if required.
​
It must be emphasised that the injection is intended to make it difficult to frown since we all frown subconsciously, this treatment prevents inadvertent frowning. Some patients will ‘test’ the effectiveness of the injection by standing in front of their mirror and trying very hard to frown! In some cases, the patient may indeed be able to use some of the frowning muscles a little if they are forced. This does not mean that the injection has not been effective.

The substance that is injected is botulinum toxin which has been in use for over 20 years as an accepted form of treatment for squints and nervous ticks. The substance cannot cause any permanent damage by unwanted travel within the body because it becomes attached to the muscle itself and thus cannot migrate. The injection technique that is used ensures that the substance cannot pass into the bloodstream. The action on the muscle is never permanent thus the patient’s frown is frozen into ‘hibernation’ for several months.
​
Due to poor tone of the muscle some older patients may feel heaviness over the eyebrows which can lasts for 2 to 3 weeks. A lower dose is therefore used in such patients to prevent this from happening. Very rarely, some patients become resistant to the drug after several treatments, i.e. it no longer works effectively. In this unlikely event alternative forms of botulinum toxin are available to avoid this problem.

RF Treatment - Radiofrequency for Facial Rejuvenation
Radiofrequency (RF) energy treatment is technology for non-surgical tightening of the early signs of loose or sagging skin; ideal for those people who either don’t want, or don’t believe they are old enough to have a surgical procedure.
Pure RF energy alone is primarily used to treat skin laxity by facial tightening and is best suited to patients with mild to moderate sagging of facial tissues, usually those in their mid 30s to 50s, with any skin colour. It is commonly used to treat the forehead, under the eyes, cheeks, mid-face, jaw line, and neck.
The use of electric currents in medicine has been documented almost since electricity itself was discovered. Radiofrequency (RF) surgery has a lengthy history of use in oral, ophthalmic, plastic, and gynaecological surgery of over 70 years. Gradually, its use in the practice of dermatology, cosmetology, cardiology, neurosurgery, hepatology, and ENT procedures gained momentum and popularity. Low frequency alternating currents are also used in physiotherapy for their ability to cause contractions in muscles. RF surgery traditionally utilizes a wave of electrons to cause frictional heating of tissues to incise, excise, ablate or coagulate the targeted tissue.

What is radiofrequency?
​
Technically speaking, radiofrequency is the number of oscillations (or waves) per second of the electric and magnetic fields within the radio waves portion of the electromagnetic spectrum. It is lowest of the electromagnetic radiation frequencies within the spectrum, which itself spans from radio waves, microwaves, infrared, optical (visible light), ultraviolet, x-rays, to gamma rays. By feeding an alternating current (AC) or voltage through an antenna or electrode, electromagnetic waves can be generated that radiate through space at the speed of light; called radio waves. Radiofrequencies, or radio waves, have wavelengths ranging from less than a centimetre to as long as 100 kilometres.

UK domestic electricity which itself is an alternating current could in theory be used for “electrosurgery”, however as most people know, contact with this kind of electric current can cause very undesirable effects such as electrocution and uncontrolled neuromuscular (nerve and muscle) stimulation. However, if the frequency of the alternating current is raised above the response time of neuromuscular structures, we find the radiofrequency range which can safely be used for medical/surgical applications.
By applying RF energy to the human body, which can conduct current and therefore act as part of the electrical circuit, any resistance encountered by the energy flow will cause heat to be produced at the site of maximum resistance, which can also be used to cut or coagulate tissue. RF energy when used for skin tightening is able to penetrate deep into the skin and affect the deeper dermis and subcutaneous layers, causing tightening and improvements to the underlying tissue structure, but with little change in skin texture or fine lines and wrinkling.

What can Radiofrequency (RF) energy treat?
​
Pure RF energy alone is primarily used to treat skin laxity by facial tightening and is best suited to patients with mild to moderate sagging of facial tissues, usually those in their mid thirties to fifties, with any skin colour. It is commonly used to treat the forehead (lift brows and tightens forehead skin), under the eyes (thickens skin and tightens and lifts the upper cheek skin), cheeks (lifts slightly and diminishes enlarged pores), mid-face, jaw line (diminishes jowling and sagging along the jaw line), and the neck (tightens skin and diminishes wrinkling of the neck).

Procedure

Your practitioner will use a Genuine Dermaroller™ medical device to produce thousands of micro-medical needle-columns in the skin. Each column will penetrate into the dermis of the skin and will be approximately 0.1mm in width. The column will close rapidly enabling the skin to recover quickly — often in the same day. Your practitioner will use a topical anaesthetic to numb the skin making the procedure comfortable; it will normally take up to 30 minutes for the topical anaesthetic to work and 20 – 30 minutes for the Genuine Dermaroller™ Therapy procedure.

After a procedure your body will start to naturally regenerate and repair the skin, working below the surface in the dermis. This takes time as new collagen is formed, new skin cells are generated and blood supply is enhanced. It can take up to 6 weeks before visible signs of regeneration and repair are seen and the process will continue over the following months, providing you with a natural and long lasting enhancement.

Your practitioner will normally recommend that your Genuine Dermaroller™ Therapy consists of a series of 2 – 3 procedures with approximately 6 weeks between them.

In the treatment of acne scars and other types of scarring it may be necessary for your practitioner to extend your Genuine Dermaroller™ Therapy.

Facial thread veins and skin tags on the body can now be removed with short wave diathermy.
​
A minute needle is inserted into the blemish (as in thread veins) to disperse the contents and destroy the tissue by thermolysis (heat). A skin tag is removed with the needle and the area will become slightly crusty as the area blackens.
​
This technique is very effective and is widely used. Sometimes additional treatments are needed.

Thread veins in the legs can be unsightly and embarrassing for some people. Now there is a treatment that will improve their appearance.

Patient Information
​
Sclerotherapy involves the use of a tiny needle to flush a small amount of a medication into the thread veins to cause them to dissolve away. We will not be able to get rid of every thread vein but we will try to decrease the venous appearance of your legs with gradual lightening of the thread veins with each session. You may feel a very slight pricking sensation as the injections are being performed.
After the first day of treatment, the skin around the vein is a little pink and blotchy. Occasionally people with more sensitive skin see more pinkness or reaction around the vein. Usually a bruise is noted at the treated site. This usually disappears after a few weeks but in some cases may take months to fade.
A length of tubigrip will be used to cover the treated area after injection. This should be left on for 24 hours. Please remember to bring the tubigrip back for use at your next session. Alternatively, you may prefer to wear your own support tights.
Do not use any creams, oils, lotions, tanning, or perfumed products on the area on the treatment day, or the day after.
Do not have a very hot bath or shower, or use any additional bath products on the treatment day, or the day after.
You should avoid prolonged sitting, pounding type of exercise, squatting and heavy weight lifting after the treatment. You are encouraged to walk after the treatment.
You are advised to avoid alcohol and aspirin 24 hours before and after treatment.
Possible complications of sclerotherapy include:

Pigmentation – a result of staining caused by small amounts of blood just
under the skin surface.
Staining – faint brown marks left by the vein walls as they are dissolving.
Telangiectatic matting – very fine small blushes of capillaries left as larger
thread veins regress.
Skin necrosis – a shallow ulcer can occur at the site of injection. This is
unusual and always heals.
Allergic reactions – very unlikely and have not been reported with the
solution used at The Medical Aesthetic Clinic.

How does the system work?
The energy from the light source is taken up by the pigments (melanin and haemoglobin) in the skin, hairs and blood vessels. Hair removal is accomplished by the generation of heat from light in the melanin, which is then transferred to the hair follicle (root). The system can be used for hair removal, vascular lesions and pigmented lesions.
Pulse times of this applied light have been chosen to destroy the hair follicle by heating for at least 1 millisecond (one thousandth of a second) to 70 degrees centigrade.

Why is gel used?
It is necessary to apply a thin layer of gel to the skin prior to treatment to ensure an optimal optical transmission of light from the applicator to the skin.

How is the treatment carried out?
The applicator (hand piece) is placed in contact with the skin surface and the light pulsed through it onto the skin. The applicator is then moved to the neighbouring area and the process is repeated.
